Add custom input box to product page

Is there a way (or an extension) to add multiple input boxes for quantity calculation to the product page?
I’m trying to create a web store for tshirts where the customer goes to the product page and can enter quantities for each size (small, medium, large) into 3 separate input boxes, then click add to cart and it calculates it into the total quantity.
Any help is greatly appreciated. This has had me stumped forever!
