Issues with the Vote button

Can someone tell me what I am doing wrong. It seems simple enough but I could be overlooking something. I did get the widget version to work. It won’t allow me to display in the side column of this page. The widget reads “no options available”.

The vote button displays a javascript error message or a line of code anyway. I have deactivated all other plugins and I get the same result.
