Format of Data Fields

Let me start by saying that Contact Form 7 works great. A little bit of thinking in setup is required, but overall, a super plugin!
Tow things I’ve noticed that I’d like some help changing:
1. In the US, an address is made up of three elements: city then a comma (,) the state (usually 2 letter format in uppercase) and finally the postcode (what we call the zipcode). An example is Shreveport, LA 71101 . What I see on the screen is: Shreveport LA, 71101 – – with the comma in the wrong place. How can the comma be positioned correctly.
2. All input fields display at a default length. How can I change the length of the input field? Ex: zipcode is 5 char length (newer length is nnnnn-nnnn); state is 2 char length, phone number is 3 char (area code) is 3 char (prefix) and 4 char (number), (ex: 318-222-5555 NOTE that dashes can be replaced with periods (318.222.5555)
3. Also, please add a postcode/zipcode field on the Event Edit form.
Any help on these admittedly small issues would be appreciated.
