Image Display

I created a form with file upload, but when I display the data, all I get is a link to the image.

How do I create a shortcode that will display the actual image as a thumbnail? I noticed on the example page that the thumbnails were showing beside the rows of data, but mine isn’t. ?

Any information would be greatly appreciated.
