Plugin that assesses answers from a questionnaire with auto reply

  • I am building a website using WordPress. Was wondering if there was a plugin, product or a way to do the following.

    Have an online assessment form that has about 50 questions and scores those questions and either sends an email stating someone is eligible or an email stating they are not eligible.

    They should be multiple choice question like responses.
