[Plugin: MailChimp List Subscribe Form] Preferred format – translations

The preferred format form fields do not update to reflect the translations from MailChimp.
A possible solution is to update the MO and PO translation files with the translated strings.
The following GitHub pull request adds in the extra fields for the English, Italian, and German translations:
