[Plugin: Events Manager] Forms not complete and thumbnail won't show

The event plugin works great with the exception of getting a thumbnail to show next to each even listing. Any ideas?

Also when clicking on an event and looking at the info, I see the booking info at the bottom. I have set the forms and info to get more information but the only thing that shows up is a comment box, no name, email, info of any sort at all.
