Follow me on a Page

  • Hi friends

    I am new to WP and using WP 3.4.1 working on my localhost.

    I would like to created a site such that every individual artist/user will login and can create a page in which their Biography with their twitter, FB and other social N/W links will display on that page(Just like Follow me). Shown in below link

    If i click on any of the social icons i should go to corresponding accounts.

    While creating a page it should ask the twitter links, FB links etc
    as shown in below link

    IS there any plugin available like this or need any hack to do this.

    Please help me.
