    Check the page using firebug to see if there are any javascript errors on the page which may be causing the problem

    I initially thought you meant that 3.4.1 was causing javascript errors – no i see you mean that there may be another plugin causing javascript errors….which in my case, there was!

    Leadology: Are you still experiencing problems? If you are, could you provide more information about them, possibly a screenshot? If not, would you like to mark the topic as resolved?

    If you use Firefox, the Screengrab (fix version) extension makes them easy, and you can host them at for example https://imgur.com.

    I’m not sure if I have the same issue, but the form doesn’t appear correctly for me either, it seems the js has stopped working (WP 3.4.1, 1.3.6 contact forms).

    I have tried firefox 14 and chrome (on a mac), they both look the same, I don’t see any js errors in the console just a million warnings (probably unrelated). Tried with IE9 on a Windows XP VM and it also looked the same as the screenshot.

    The form submits to /wp-content/plugins/slick-contact-forms/slick_mail.php, but not in a pretty ajaxy way, it just takes over the whole window. I tried clearing cache, etc, but still looked like the screenshot.

    As the plugin is working well on my 3.4.1, the issue might be related to plugin or theme conflict. If you could try with the default theme and with all other plugins disabled, then turn them on one at a time, and last re-enable your current theme, the cause might be found.

    It’s good to reserve some 15-30 minutes for doing the above, and to take a backup in beforehand in case some plugins lose their settings on disabling and the exact settings are hard to remember.

    I am not sure If I am posting in the correct forum thread but I am having trouble with my Slick contact form and it displaying differently on different operating systems. I am creating a website on a Mac OS X Lion 10.7.4, and the slick contact form displays just as I want it. However when I looked to check the form on a windows operated PC the form was just a white box blocking the content of my homepage?

    Any ideas?

    Adventure Eyes: Do you have some custom CSS or JS affecting the display of the plugin? In other words, are you experiencing this on a vanilla WordPress install (default 2011 theme, no other plugins active)? Which browsers do you use on each OS?
