[Plugin: Fast Secure Contact Form] Contact Form Is Not Sending To Web email address

The contact form is configured to send email notification to the email address associated with the domain name. The “Custom E-mail From” has the same domain name but different name. For example [email protected] is the email to address and [email protected] is the from address. The “Enable when web host requires “Mail From” strictly tied to domain email account” box is checked
When the contact form is completed on the site there is an email sent to the email address associated with the initial setup of the site. It does NOT send the contact information to the email in the form.
The test email does go to the email address in the form.
I have followed all the FAQ solutions for getting this fixed and including turning off all the plugins. The contact form is not sending to the email address configured in the form when completed on the site.
Please advise.
