Post Notification AND Lead Capture

I am looking for ideas on the most elegant solution to the following.

1. I want to provide a lead capture form and save those leads to a third party CRM such as mailchimp or salesforce.

2. I also want to send post notifications to those who fill out the lead form.

I have found addins that do each of these things, but haven’t seen any that do both.
