[Plugin: WordPress-to-lead for Salesforce CRM] No Salesforce widget

1) Salesforce Widget is missing
2) Shortcode results in “sidebar widget” appearing in the page/post i.e it does not display the main form as it should on a page.
3) Shortcode only works with forms = “1” or similiar
