Facebook Like Button Shows Wrong Like Count On Home Page – HELP!

Hello WordPress!

We’re having a problem with Facebook like button and other social share buttons as they are show wrong counting. The problem is only being faced when the code is put in “index.php” to appear on Home page. Else on posts the count is working fine.

We are using the code provided by AddThis, here’s it:
