web form to automaticaly populate registers – which is best?

Hi there, any help is gratefully received ??

I’m trying to create a web form so visitors can book onto courses online, and then my registers are populated automatically.

I have around 6 fields that would need drop-down menus customisable by me for users to select and then press “book”. An automated email would then be sent out, and their data captured in my database, which then automatically populates registers for me (in word or excel).

Does anyone know how to do this or a suitable plug-in please?

I’m using child theme of twentyeleven, and my website is https://www.icanplaytennis.net – it is the data from the coaching sessions which I would like to be allowable to book online!
