Did you try a search for survey and/or poll?

A survey plugin is about to exist — I’m finalizing it and will be releasing it at the end of the month.

Any update on this plugin? I’ve followed the links you’ve provided and really wasn’t presented with any useful info. i.e. — a download, a status, a working example, etc.

I am still looking for a survey plug-in. Since this would be for personal use, I am not interested in a plug-in that I have to pay for.

You might have a look at our new WordPress survey plugin – SurveyGizmo. It allows you to embed a VERY full featured survey tool into WordPress.
