The only thing I can think of at the moment is somehow the install/activation process didn’t run properly and perhaps you are missing the intrusions table in your database.

Check to see if you have a table in your database named “wp_mscr_intrusions”, this is where the intrusions are stored. If the “wp_mscr_intrusions” table already exists then we’ll have to keep digging to find the problem.

If the table is missing and you want to re-create it just de-activate and then re-activate Mute Screamer. The activation process will create the table if it does not exist.
