How to add an extensive form to a page

    I need to add a form for potential volunteers to one of my sites. I use Ryan’s contact form (it’s great!) for gathering basic information. But, I need to use drop down boxes, radio and check boxes, have a preview of the form and also send a copy to me and the person filling it out. I’d also like it to post to a database. I tried using the Run PHP plugin, but the code still shows up in the page. (I generally have trouble using this plugin anyway.)

    Has anyone done this? I sure would appreciate the help!

  • No, I haven’t done this, but I’d go in this way:
    – Create a page template where you put all the code you need
    – create a new Page using this particular template and do not put anything in the content area
    – never use this Page template for anything else
    (I don’t know anything about the part of posting to a db)
