Custom form and reply

  • I need your help. I have a site where you can download a file but first I want to get some info and then send the user a reply to the email address they entered with a link to the file.

    Can someone recommend a Contact form widget or plugin that will

    1. That is easy to setup use
    2. Will allow me to create at least 7 custom questions and make them mandatory (one of which will be the user’s email)
    3. WIll allow me to reply to the USER’s email they just answered with the download info.

    I have tried a few but not one that handles #3 requirement – send email back to user
