[Plugin: Theme My Login] Insertion Error: Duplicate entry

I am using MU and when I try to create a site I get an error:

Insertion Error: Duplicate entry ‘theme_my_login’ for key ‘option_name’ – The template was not applied. (New Blog Templates – While inserting templated settings)

I checked my plugins list in network admin and there were two instances of “theme my login”.
I removed one and I still get the error.

ANy ideas please help
