Contact form to pull email from custom field

Hello WP Forum,

Goal:

To create a contact form without the use of a plugin, that will allow me to change the following via an options panel or the custom fields of the contact page.

a) Email Address (recipient)
b) Auto Responder Message

Progress

Contact form complete (using an external .php file)

Issue

I have no idea on how to proceed to the next step, how would i make the contact form pull the meta data from the page contact ?

The form would only submit from the one page, so the page will be active whilst the form is submitted.
