Needing Help WIth A Specific Contact Form Plugin

I found a pop up contact plug-in online that I would like to use but not sure if they changed the name of the plugin cause I cannot find it on WordPress. It was called Contact-pop and you could cutomize the options. I need an option for a guest to select the state they are from. Looks confusing to me so I was hoping I could just add the plugin on wordpress without the hassle.
