Recommend multiemail contact form

I need to setup a questionary form on the site with around 50-70 fields to fill it or choose from options list, and then I need to email filled form to different email addresses depending on selected options in one of the lists. Also I need to send one copy of form to main email and one copy to newsletter subscription email (only if user agrees to subscribe) I also want to set captcha for this form.
