You have a lot of Javascript errors on your password-reset page.
Look at your Browser web Console for the JavaScript errors.
Mouse right button click, select Inspect then Console in the Menu.

Try to do a Theme/Plugin conflict test if your Theme
or another Plugin is causing a conflict with UM.
