This problem may be related to other plugin used in your shop. Please try to isolate an issue, by disable plugins which are not our or WooCommerce. Then please enable plugins one by one, deleting cache, and check if the problem still exist.

If yes, please test the plugin again, but now using Storefront – official WooCommerce theme. It will help us identify a possible conflict with your theme.

Hi @opvelp,

I would like to ask if you may have tested a potential plugin conflict – as I recommended in the message above? Additionally, you can try going to WooCommerce → Order Fields and for the section where the problem is occurring do a “Section Settings Reset” this will remove the added fields using the FCF plugin, however in some cases this resolves the fields working properly.
