I’ve visited the page and even before I tried “Choose files” link, I noticed a lot of JS errors (related to Forminator scripts) in browser console.
Such errors are usually related to some sort of JS optimization applied to the site or to unexpected conflict (with theme or other plugin) or both.
Are you using any plugin that optimizes JS delivery, for example delays/defers it? If yes, could you try disabling such options, purge all cache and see if that helps?
If that’s not the case or it doesn’t help, a full conflict test would be necessary to identify what’s causing the conflict:
– temporarily switch site to Twenty Twenty-Four theme
– and disable all other plugins except Forminator
The issue should be gone at this point so then enable original theme back and check again. If the issue is back, it means conflict is with theme so let us know about it and you can skip the rest of the test (and enable all you necessary plugins back).
If it’s not back, start enabling plugins back one-by-one, after each one checking for the issue. Once it’s back, it means that last enabled plugin is the cause of the conflict so knowing about it we could dig into it to find out why it’s happening and how to fix it.
