Mask don’t show correctly

Hi. I have some problem with phone mask on https://fortis.world website (block “Talk with our expert” on bottom of page)
1) It’s only show Placeholder, and mask appear again when you start typing. Sometimes while page loading we can see mask for couple seconds, and after that it’s disappear. How can I show mask everytime?
2) On mobile same problem, but also mask don’t appear even if we start typing. That’s why I getting garbage value with no mask when form submitted. How can I fix this?
