URL Parameters Acting Strangely

So I have a form that processes through a 3rd party API plugin into Zapier and then on to the client’s CRM. Part of this process if collecting UTM data for marketing purposes, but for some reason I just started getting errors out of nowhere.

If you go to the page in the link provided, you’ll see two iterations of the same exact form. The fields on top are pulling parameters from the URL. Only thing is, in one form, I get a full URL as a parameter, and in the other form (same exact form in the backend) I get the actual parameter I’m looking for. Does anybody know what could be causing this?

Edit: Obviously, these will be hidden fields in final implementation, but this was the easiest/quickest way to troubleshoot.
