To adjust the layout of the mentioned fields, please add the following code to the Additional CSS editor within the Customizer.

.form-group.ppom-input-dedykacja_ .form-control-label,
.form-group.ppom-input-dedykacja_ .form-check-inline {
    display: block;
}
