Tracking number entries are creating 2 lines of the same tracking number

when i enter the tracking number for Australia post into the order screen , it creates 2 entries in the shipment tracking widget, order notes and the email to customer. it also completes the order but quickly reverts back to previous status like ón hold’. i cant figure out why this is happening. see screenshots for reference.
