Avatar won’t change

We have been using this plugin for several years. Recently we changed the default avatar to a new image. But in the user list, some users still have the old default avatar. When I attempt to change the avatar to a new image, the old one still remains. I have cleared the browser cache and the site does not use any other caching plugin.

Any ideas on what to try next?
