How to create newsletter with my own subscription form

Hello everyone,
I’m looking to create a newsletter with my own subscription form with the following fields: Name, email, date of birth and phone number. But I did not find any plugins free that can do that. Do you have a solution? Thank you in advance and sorry for my English.
