Adding a Javascript calculator form to a single page…

Hello,

I have a site that I’m redesigning using WordPress and Thesis, but one thing I would like to incorporate is the existing calculator form that calls an external js file. So far I have added the form (and tried adding the .js script call) directly into the page. It doesn’t work, and I didn’t think it would, as I probably have to add this into custom_functions as well? Sorry, I’m not much of a developer, I just want to add this to a single page.
