Payment processed but order not added to system

I have a WooCommerce problem that I’m just not sure how to debug. Any ideas or direction are appreciated.
The store is basically working. A customer can place an order and pay for it. The payment is processed and the notification emails are sent out to the store admin and the customer. The payment is deposited to the checking account and shows on the DEPOSIT page under PAYMENTS.
What does NOT happen is the order information does not appear to be entered into the database. There is no sign of the order on the order dashboard. On the TRANSACTIONS page the order is listed but in the order number column there is an N/A. The order ID/number referenced on the confirmation emails is goes to some other site asset, i.e. the website logo. No order ID is created.
WordPress, the theme (DIVI), all the plugins, and PHP are all up to date.
The store is not the main purpose of the site so we only get a few orders here and there. As a result we didn’t notice the problem right away. The first instance happened back in November. Everything was working well prior to that and we’ve made no changes other than regular version updates.
Any thoughts or direction on how to debug this problem and find where the breakdown is happening?
