Simple Contact Us From Plugin?

I have been searching far and wide for a plugin that is a simple Contact Us form that can be put into it’s own page. I have tried many different ones and nothing seems to work.

Currently I am using Free Hosting for my blog and need one that works without using any internal server settings and just works ‘out of the box’.

Any help in the right direction is appreciated! Thanks!
