Certain fields not mapping Web-to-Case

    Hi,

    I am facing an interesting issue. We have created a form and mapped fields with SalesForce. Fileds are a mixture of standard and custom fields. However, only part of them are syncing to SalesForce. Maybe you have any ideas where is a problem?

    Fields in Question:

    Field Label: Web Phone
    Field Name: SuppliedPhone
    Field Type: text

    Field Label: Due Date
    Field Name: Due_Date__c
    Field Type: date

    Field Label: Subject
    Field Name: Subject
    Field Type: text

    p.s. regarding date field, I made sure the date formating is correct with Locale in Salesforce.
