[Plugin: Meenews] Where to put Subscription form

I just bougth the meenews WordPress plugin and can’t find out where to put the code for the subscription form.

As i think it’s this:

					<?php TvNewsletter::activateNewsletterPlugin(); ?>
					Das ist Text
					<?php endif; ?>

I tried to copy it in a custom template but nothing shows up. Anybody an idea, how to get the subscrition form in a custom template or even a post or page?
