User Upload Plugin?

I am looking for a plugin that duplicates something similar to the Failblog uploader. Anyone have any experience with one? The uploader can be seen at https://cheezburger.com/fail.aspx

Basically, I something that will enable me to provide a place for users to upload pics and caption them. These would ideally then be stored for my review/ approval before posting.

I realize I can do this through WP out of the box, but I find it clunky.
