Eu cookie bar issue

Hi. I set up eu cookie bar but these setting are showing correctly only on 2 pages:
https://apartamentlux.pl/galeria/https://apartamentlux.pl/rezerwacja/
on the other 2 pages is not display correctly. Cookies settings are blank.
I don’t know where is the problem?
