• I am trying to create an online membership application form for our organization, but have run into a section of the application that I can’t figure out how to code in CF7. This section of the application requires that the applicant choose 1 of 3 ways that we can “vet” their application (and the 3rd option has 2 variations):

    On the written application form, we state it like this:

    Please provide ONE of the following

    • `
      A Signed affidavit of five (5) or more current TxAPBR Colony Removal Specialist members. Affidavit must include the length of time the member has known the applicant as well as a testament to the professionalism of the applicant’s removal skills.
    • Signed letters of reference from a minimum of five (5) colony removal customers, including pictures or video of the removal.
    • Been in the bee removal business for a minimum of 3 years and either:
    1. signed affidavits from five (5) or more other well-known & respected bee removal
      professionals in Texas.
    2. – or –

    3. The names of 2 or more current TxAPBR Board of Directors who will attest to your
      professionalism and experience

    Obviously if the person chooses the first, second, or 1st half of the 3rd option, they’ll have to send those affidavits separately.

    If the person chooses the second part of the 3rd option, I would like to just give them a textarea where they can write those names.
