I ended up using a different form because I couldn’t get this one to work. This is the url that I have when I have the form pulled up on WPlite https://simplyembellishedlife.com/wp-admin/admin.php?page=wpforms-builder&view=fields&form_id=1547
Even though I used something else, I’d like to know why this one didn’t work if possible. Thanks for replying

Thanks for your response.

The link you have provided is the link to the form builder which needs someone to be logged in to the site to see it.

It’s against www.remarpro.com support policy to get access for this but if you can temporarily place the form on the page and give us the link to it, we can check and have a look.
