Plugon withdrawn from WordPress

Dear all,
The plugin has disappeared from my extensions in the back office of my wordpress website. It is critical since I use these forms to provide questionnaires to my clients.
I do not understand why it disappeared and I fear all the settings have to be reset with the plugin reinstallation from new. Can I just install the backup plugin? I am not sure the settings are coded in it…

Thank you for your comment @jefromcanada. But do I need to reinstall the plugin from scratch and rebuild the forms in the end? Do you know this?
