Custom form fields Short code for notification email

  • Hi,
    I created several custom filed and I wanted to have these filed included in customer notification email as well as one sent to assistant.

    I tried using label but with no success. What should I do.

    By default all the custom fields are displayed inside the email notification.

    There are not showing here. How to know the custom field short code.
    [NAME] [DATE] [TIME} are shown, but none of the custom field. How to know their name? Is there documentation about that?
