Embedded form does not display fully

Hi, I have embedded a 3rd party form in an elementor pop up using the elementor html widget. It was all working fine until july 15. Now the form does not fully display. If you click on the link I provided and click on “free download” you will see that only the first two form fields aspire displayed. For some reason elementor is not displaying the full form. I have submitted a ticket but not had a response in 3 days.
