Empty field for radio button

    Dear Sir / Madam,

    Sometimes when I summit the form below, it will not send the “locatie” field (radio button) to the back-end / mail. In the back-end I see an empty field for the “locatie” field (its required so I did select one option).

    I already remove all caches.
