I use the WP Amelia plugin for this. It’s an all-in-one solution, but a commercial plugin. I find the $59 well worth it, compared with the time and effort that I used to spend in coupling together different free plugins to achieve the same result.

That said, there are several free appointment booking plugins out there.

Account creation is a built-in feature of WordPress.
