Thanks for sharing the screenshot. Generally, the duplicate form submission is caused by Safari on iOS (iPhone, iPad). The users submit a form entry and either go to another app or open a new tab. At a later point in time Safari is reopened or the tab is brought to focus and then Safari refreshes the page as it deems it is “stale”. When Safari does this it triggers another submission. We’re currently looking into this issue and should get fixed in one of the future releases.

In the meantime, you may consider enabling AJAX submission. Or, you can redirect the users to the thank you page.

Well, I am not using Safari at all. I tried it with Firefox and Chrome, so not quite sure if thats the problem!?

I will give this AJAX thing a try.
