Plugin Caused Inability to Enter Text Into Forms

I had to turn the plugin off today while troubleshooting why text was not being allowed to be entered into text forms. I realized this plugin seemed to be the cause of the problem. I have deactivated and even re-installed the plugin with no luck in solving the issue. I am wondering if there is another approach I should take in resolving this issue.
