Columns not level

It has taken time to find, but I have worked out how to put 2 fields in a row, which is great, but I have an issue and a question.
When I put 2 sets of 2 fields in a two rows, the second column is not level. Also, the data boxes are smaller on the second set than the first (they are all set to small).
Second, how do I put a field into column 2 without having something to ‘match’ it in column 1 i.e. I want column 1 empty.

The plugin includes multiple options to distribute the fields in columns using container fields or by assigning the predefined class names directly to the fields.

For example, to split the row into two columns and display the field at right with the left column empty, you can assign to this field the class names:
