    Dear user, there is a conflict between the Form Maker and your theme or other plugins you use.

    Please install the Health Check plugin and try to understand which plugin/theme gives the conflict.

    Thanks for your reply. The conflict appears to be with the Hello Elementor theme. However, changing to a default WordPress theme negatively impacts the layout of large parts of the site. Is there any fix for the conflict with the Hello Elementor theme?

    We need a link to the form page to be able to advise a solution.

    You can create a password-protected page and send the link along with the password to check the issue.

    Solution for other users:

    There is a style conflict with the theme, please add the below code in Appearance > Customize > additional CSS section and the issue will be solved.

    .ui-datepicker-title select {
    display:inline-block !important;
    padding: inherit !important;
    }
