Quiz not showing and error on submit

    Two issues:
    1. Quiz does not display
    <label> Your Name (required)

    <label> Subject

    [submit “Send”]

    2. On send, page changes to just show ‘0’ and URL changes to

    PHP 7.2.25
    Phlox Theme
    Contact Form 5.3.2
    Elementor 3.1.1
    Flamingo 2.2.1
    Polylang 2.9.2
    Sucuri Security 1.8.25
    Phlox Core Elements Plugin DISABLED as it stops menus displaying

    Try to replace your Form tab content for this one (copy & paste):

    <label> Your Name (required)
  [text* your-name] </label>
<label> Your Email (required)
  [email* your-email] </label>
<label> Subject
  [text your-subject] </label>
<label> Your Message
  [textarea your-message] </label>

    After updating the code, try to send a message through your contact form.

    Thanks.
    This in itself did not make any difference – I put ‘Your New Name’ in place of ‘Your Name’ so I could see whether the change had taken effect, and it did not show.
    So I deleted the block with the form in in Elementor, and added the [contact-form ......] text in a new block, and it worked fine.
    I think it must have been some Elementor upgrade issue or something.
    I did the same with the French version of the form, where I did not change the text at all, and it also worked again.
